How do I get a copy of my medical record?

Fill out an
Authorization Form
and return it to the HIM Department listed above or turn in at any Valley Mental Health location, and they will forward the form.
|
|
|
|
|
You may also request to review your records with your therapist by scheduling an appointment.
|
|
|
|
|
If you are the parent or guardian of a child in alcohol and/or drug treatment, your minor child must also sign the request form.
|
|
|
|
|
Your request will be reviewed and approved by your therapist. Please allow time for this process.
|
|
|
|
|
There may be a charge for copies.

Ask for a form at any Valley Mental Health location.
|
|

|
Request a copy of the form by contacting the HIM Department by phone, walk-in, or email provided above.
|

Read and fill in all sections of the authorization form then sign and date.
|
|
|
|
|
|

The completed authorization form can be returned to the HIM Department a number of different ways:

Visit any Valley Mental Health location and they will assist you with sending in the completed form to the HIM Department.
|
|
|
|
|
|
|
|
In person, by email or fax at the HIM Department; found in Contact Information above.
|
|
|
|
|
|
|
|
Please bring a photo ID with you when dropping off or filling out an Authorization form.
|
|
|
|
|
|
|
|
When picking up your records, a photo ID must be presented.
